Home Based Self Directed Assistance
New Horizon Center offers Home Base Service Facilitation (HBS) for individuals who have been approved for the Home Base Program. The self-directed assistant works with families and individuals to assist with:
- SSI benefits
- Medicaid issues
- Personal support workers (PSW) tasks
- Funding allocation
- Home modification and adaptive equipment
- Access financial management
- Service agreements
- Employer of record
- Review of a personal plan
- Seeking residential placement
- Acquiring technology equipment
